Pre-Employment Services: What to expect during your visit
Integrated Rehab is proud to be listed on the U.S. Department of Transportation Federal Motor Carrier Safety Administration’s National Registry of Certified Medical Examiners.
Be prepared for the following when you come to your appointment:
Drug Screen:
- Please bring a valid state-issued photo ID.
- You will complete a Demographic intake form (or print from our website and bring with you)
- Please come prepared to provide a minimum of 45 mL urine sample.
- You will be asked to empty your pockets, remove outer garments (i.e. jackets, sweater, hats, hoodies etc. ).
- You will then required to wash your hands on site.
- Once you are given the specimen cup, you will provide your specimen and return to the collector in our office.
- You and the collector will complete Chain of Custody with signatures, labels and dates. Then your visit is complete.

Physical:
- Please bring a valid state-issued photo ID.
- You will complete a Demographic intake form (or print from our website and bring with you).
- You will complete the physical health history portion. Please bring a list of all medications.
- You will be asked to provide a urine sample to check blood, sugar, protein and specific gravity.
- Basic vitals including a vision and whisper test will be completed.
- Doctor will complete exam.
Physical Capacity Profile:
- Please wear comfortable clothing and shoes.
- Please bring a valid state-issued photo ID.
- You will complete a Demographic intake form (or print from our website and bring with you).
- You will complete a health history form.
- Health history from will be reviewed with the team member completing your PCP.
- Your test will take approximately 30 minutes from start to finish
